Targeted Homeless Case Management provides case management services to individuals who are homeless in order to support them with accessing needed services including, Medical insurance, Primary care providers, mental health supports, Transportation, Disability benefits, etc.

A sample of Provided Services:

• Referral to primary care providers
• Referral to mental health services
• Assistance with applying for benefits, Assistance applying for MassHealth and food stamps
• Establishing a Plan of Care
• Insuring services are being provided by monitoring outcomes
