HIV CareConnect

 

HIV CareConnect provides comprehensive, whole-person support for individuals living with HIV. The program coordinates medical care, behavioral health services, and social resources to ensure patients have access to the care and support they need. Our team works closely with patients to navigate healthcare systems, promote wellness, and foster long-term health and stability.


  • Medical Care Coordination – Connect patients with HIV-specialized healthcare providers and manage treatment plans.
  • Behavioral Health Support – Counseling, therapy, and mental health services.
  • Substance Use Support – Access to recovery resources and programs for substance use management.
  • Case Management – Personalized guidance navigating healthcare systems, appointments, and treatment adherence.
  • Medication Management – Assistance with prescriptions, adherence, and side-effect monitoring.
  • Social Services Assistance – Help accessing housing, transportation, nutrition, and other community resources.
  • Health Education – Information on HIV care, prevention, and wellness practices.
  • Wellness Programs – Initiatives to promote physical, emotional, and behavioral well-being.
  • Patient Advocacy – Support patients in understanding their rights and accessing benefits.
  • Peer Support & Mentorship – Connection with peers for guidance, encouragement, and shared experiences.
  • Discharge & Transition Planning – Coordinated support when transitioning from inpatient care or other programs.
  • Community Resource Referrals – Linkage to legal, vocational, and other social services.
  • Outreach & Engagement – Proactive engagement with patients to ensure ongoing connection to care and support.

Weather Closure Notice – Tuesday, December 2, 2025

Due to the forecasted weather conditions, all Worcester and Boston area offices will be closed on Tuesday, December 2, 2025.  


  • There will be no in-person case management appointments and no home visits.  
  • All scheduled appointments will be conducted via telehealth. At your appointment time, please log in as usual and your Case Manager will meet with you.  


Transportation services will also be suspended. There will be no transportation provided to non-urgent medical appointments.
