TransHealth Link

TransHealth Link provides comprehensive, affirming case management for transgender and gender-diverse individuals. The program connects participants with medical, behavioral, and social support services while maintaining the highest standards of confidentiality and respect. Our team works closely with individuals to navigate healthcare systems, access resources, and promote overall well-being and long-term stability.


Services Offered

  • Affirming Care Planning – Develop personalized case management plans that respect each individual’s identity and needs.
  • Medical & Behavioral Health Coordination – Connect participants with trans-competent healthcare and mental health providers.
  • Peer Support & Mentorship – Facilitate confidential connections with trained peers or community mentors for guidance and encouragement.
  • Resource Navigation – Assist with accessing housing, employment, legal support, and community programs.
  • Appointment Support – Help schedule, remind, and follow up on medical or behavioral health visits confidentially.
  • Treatment & Goal Monitoring – Track progress toward health and wellness goals while maintaining privacy.
  • System Advocacy – Act as a liaison between participants and providers to ensure respectful, coordinated care.
  • Crisis Support Coordination – Ensure timely access to urgent or emergency services when needed.
  • Education & Wellness Workshops – Offer peer-led or case manager-guided sessions on coping strategies, stress management, and resilience.
  • Follow-Up & Continuity – Maintain ongoing support to ensure long-term health, engagement, and stability.

Weather Closure Notice – Tuesday, December 2, 2025

Due to the forecasted weather conditions, all Worcester and Boston area offices will be closed on Tuesday, December 2, 2025.  


  • There will be no in-person case management appointments and no home visits.  
  • All scheduled appointments will be conducted via telehealth. At your appointment time, please log in as usual and your Case Manager will meet with you.  


Transportation services will also be suspended. There will be no transportation provided to non-urgent medical appointments.
