Homeless CareLink

Homeless CareLink provides comprehensive, whole-person support for individuals experiencing homelessness. The program connects participants with medical care, behavioral health services, and essential social resources to address their immediate and long-term needs. Our team works directly in the community to remove barriers to care, promote wellness, and support stability and self-sufficiency.


  • Medical Care Coordination – Connect participants with healthcare providers and manage ongoing medical needs.
  • Behavioral Health Support – Access to mental health counseling, therapy, and substance use services.
  • Case Management – Personalized support to navigate healthcare systems and social services.
  • Housing Assistance – Help locating and securing temporary or permanent housing options.
  • Emergency Support Services – Access to food, clothing, hygiene supplies, and crisis resources.
  • Health Education & Wellness – Programs on prevention, healthy living, and managing chronic conditions.
  • Medication Management – Assistance with prescriptions, adherence, and side-effect monitoring.
  • Discharge & Transition Planning – Coordinated support for participants transitioning from shelters, hospitals, or other programs.
  • Social Services Referrals – Connection to employment, vocational training, legal aid, and community resources.
  • Outreach & Engagement – Proactive engagement in shelters, streets, and community locations to connect individuals to services.
  • Peer Support & Mentorship – Peer guidance to encourage engagement, recovery, and community reintegration.
  • Patient Advocacy – Empower participants to understand their rights, access benefits, and navigate systems.
  • Follow-Up & Care Continuity – Ongoing support to ensure long-term stability, adherence, and wellness.

CareLink Meals

 CareLink Meals provides warm, nutritious weekly meals to individuals experiencing homelessness across the Commonwealth. Rooted in compassion and community partnership, the program ensures that people facing food insecurity receive consistent nourishment, dignity, and connection.


Each week, our team—alongside volunteers and community partners—delivers freshly prepared meals at accessible locations, meeting people where they are. CareLink Meals is designed to reduce hunger, build trust, and serve as a bridge to essential health and social services offered through United Care of Massachusetts.


Beyond providing food, the program fosters relationships that support long-term stability. During meal distribution, participants can connect with Case Managers, receive resource information, and be linked to medical care, behavioral health services, housing assistance, and other supports.


CareLink Meals is more than a meal—it’s a pathway to wellness, hope, and community connection for our neighbors most in need.

CareLink Closet

CareLink Closet offers free clothing, shoes, and essential personal items to men and women experiencing homelessness or extreme financial hardship. 


Our program ensures everyone has access to clean, seasonally appropriate clothing, footwear, and personal care items in a safe, welcoming environment.


The Closet also provides hygiene kits, blankets, and weather-appropriate gear, helping individuals maintain dignity and confidence while navigating daily challenges. During visits, participants can connect with United Care of Massachusetts for case management, medical care, behavioral health support, housing assistance, and other community resources.


CareLink Closet is more than clothing—it’s warmth, confidence, and a bridge to stability and long-term support.

Weather Closure Notice – Tuesday, December 2, 2025

Due to the forecasted weather conditions, all Worcester and Boston area offices will be closed on Tuesday, December 2, 2025.  


  • There will be no in-person case management appointments and no home visits.  
  • All scheduled appointments will be conducted via telehealth. At your appointment time, please log in as usual and your Case Manager will meet with you.  


Transportation services will also be suspended. There will be no transportation provided to non-urgent medical appointments.
